Actualmente estoy aprendiendo un poco sobre edición de imágenes y diseño gráfico, he descubierto que me gusta mucho, así que si alguien tiene un buen consejo al respecto, lo leo feliz. Me gusta leer novelas (me declaro fanática de las novelas de amor). No apoyo la idea de encasillar a la gente, ya sea por ideologías, religión o clase social, creo que esta es una manera de coartar el crecimiento de cada persona. <h2>Apuesto ciegamente a la educación como herramienta para mejorar sociedades.</h2>\n \n\nHace unos años cuando me preguntaban a qué quería dedicar mi vida la respuesta siempre era “ayudar a las personas”, solo que no sabía cómo. Luego de pensarlo bastante (unido a mucha presión social), decidí comenzar a estudiar medicina… y la verdad es que me enamoré de esta profesión.\n\n\nA pesar de vivir en un país donde los hospitales no funcionan al 100% y donde hay que batallar diariamente para conseguir los insumos necesarios para atender a los pacientes, no hay nada mejor que sentir que realmente estás mejorando la vida de alguien, con esa sensación podría vivir todos los días. Desde esta plataforma digital buscamos ayudar a todos los estudiantes de medicina, y del área de salud en general, a que entiendan que lo que leen en los libros pasa en la vida real, mostrando casos clínicos que nosotros mismos hemos visto, para que entiendan realmente la importancia de aplicar sus conocimientos en su área de trabajo. \n\n\nDesde <strong>Medicitosis</strong>, apostamos por una manera diferente de aprender sobre la salud. Estar encima de una tarima no tiene comparación, los nervios antes de salir y la emoción que sientes cuando finaliza la obra y vienen los aplausos. Me declaro fiel amante de la libertad, lo cual es irónico al vivir en un país donde hay tantas censuras, aunque quizás por eso he aprendido a valorarla tanto, dicen que uno valora más las cosas luego perderlas.       "body": "<strong>ENFERMEDAD ACTUAL</strong>\nPaciente masculino de 58 años de edad con antecedente de aumento de volumen en región inguinal derecha desde los 8 años, el cual reducía espontáneamente, quien refiere inicio de enfermedad actual hace 1 mes cuando mismo aumento de volumen se mantiene constante (irreductible), asociándose dolor punzante, de leve intensidad, intermitente y que se exacerba con el esfuerzo físico, progresando a  fuerte intensidad sin periodos de acalmia siete días previo a evaluación, concomitante vómitos de contenido y frecuencia no precisados e hiporexia, motivo por el cual acude y se admite.\n\nAl momento de su ingreso, el paciente presenta aumento de volumen en el testículo derecho, no reductible, saco doloroso a la palpación. Se evidencian cambios en la pigmentación del tejido, sugestivos de isquemia intestinal, lo cual ocasionó la necrosis de éste; además de olor fétido. No se realiza maniobra de Landivar, ya que la hernia es irreductible.\n\nSe le ordena tratamiento quirúrgico, con abordaje abierto, durante el cual se halla la presencia de una porción colon dentro del saco herniario en el testículo derecho. Imagen con difusión autorizada por el paciente\n\n<strong>DIAGNÓSTICO</strong>\nSe trata de paciente con hernia inguinal derecha estrangulada, la cual se complicó con la salida de una porción del colon al testículo derecho.\n\nUna hernia es la protrusión o salida al exterior del contenido de una cavidad, generalmente a través de un orificio natural o de una zona de debilidad de la pared que lo contiene. Las hernias inguinales, representan el 75% de todas las hernias, siendo más comunes en hombres. Se pueden clasificar en directas (ubicadas dentro de los vasos umbilicales obliterados) e indirectas (ubicadas fuera de los vasos epigástricos inferiores). Las hernias inguinales directas son de carácter circular, generalmente no poseen gran tamaño, y sin contenido visceral. Mientras que, las hernias inguinales indirectas, como la del paciente, son de carácter ovoide, de gran tamaño, suelen tener contenido visceral y con tendencia a dirigirse hacia el escroto, debido a la persistencia del conducto peritoneo vaginal. Al examen físico se diferencian mediante la maniobra de Landivar, la cual se realiza ocluyendo el orificio interno del conducto inguinal con el dedo del examinador, luego se le solicita al paciente realizar una maniobra de Valsalva (aumentar presión intraabdominal al toser o incorporarse), si aparece la tumoración, la hernia es directa; si no aparece la tumoración, con seguridad es indirecta.\n\nUna hernia puede complicarse, convirtiéndose en una hernia encarcelada (crónica), atascada (aguda) y, como es en el caso del paciente, estrangulada (aguda). Las hernias estranguladas son aquellas en las ocurre un daño en las estructuras intestinales por una compresión de los tejidos que dificulta la vascularización de éstos, pudiendo provocar isquemia y posterior necrosis del contenido herniario.\n\n<strong>¿CÓMO RECONOZCO QUE MI PACIENTE TIENE UNA HERNIA ESTRANGULADA?</strong>\nSe presenta dolor de intensidad moderada durante la palpación de la hernia a través de la piel, el cual puede ceder durante las primeras horas con la ingesta de AINES. Se pueden producir náuseas y vómitos en relación con los episodios de dolor. Aunado a esto, se evidencia tumoración irreductible e incluso bloqueo del tránsito intestinal.\n\nSi la hernia inguinal estrangulada prosigue en su desarrollo, es posible que los tejidos que han quedado atrapados en el orificio herniario puedan sufrir un deterioro importante, provocando una isquemia intestinal que puede conducir a la necrosis del tejido y la rotura de parte de ese contenido intestinal, tal como sucedió en el caso de este paciente.\n\nYa que estamos en presencia de un paciente con una herida sucia (traumática con tejido desvitalizado, edematizado y vísceras huecas perforadas), existe un riesgo mucho mayor de que se presente una infección postquirúrgica, como sucede en el caso de este paciente, razón por la cual el paciente se mantiene internado y con atención médica permanente.\n\nEl tratamiento es netamente quirúrgico. Consiste en la reintroducción del contenido abdominal y reparación o reforzamiento de la pared inguinal. Para la realización de dicha reparación o herniorrafia, existen varias técnicas que van desde la utilización de los propios tejidos (músculos, fascias, etc.) para restaurar la integridad de la pared abdominal, hasta la utilización de prótesis o mallas artificiales que ejerzan dicho papel.\n\nLas imágenes mostradas fueron tomadas luego de la resolución quirúrgica de la hernia, durante el proceso de curación.\n\nActualmente el paciente se encuentra con un pronóstico favorable / evolución satisfactoria y a la espera de alta médica, para continuar su tratamiento.\n\n\n<strong>Dicho​ ​caso​ ​es​ ​presentado​ ​con​ ​previo​ ​consentimiento​ ​del​ ​paciente.</strong>",
