With patients <a href=\"https://www.washingtonpost.com/news/to-your-health/wp/2018/03/02/doctors-started-brain-surgery-then-realized-they-were-operating-on-the-wrong-patient/?noredirect=on&amp;utm_term=.e9b2e7084b3c\">receiving the wrong brain surgery</a> and <a href=\"https://www.theverge.com/2017/5/12/15630354/nhs-hospitals-ransomware-hack-wannacry-bitcoin\">hospitals' data infrastructures being hacked globally</a>, this sector is in need of a serious technological update.  Highly sensitive information such as patient lab records or patient vitals data can be compromised by data breaches.\n\nThese data security issues plague healthcare providers, health insurance companies, and patients.  Blockchain technology can act as a secure reservoir for all healthcare data that feeds all the participants of the healthcare system.  The blockchain can secure patients' lab records, biometrics, insurance claims, and fitness data.\n<h2>The problems</h2>\n\n<hr />\n\n<b>Everything has gotten increasingly connected - but can we deal with the demand and security risks?</b>\n\nHospital devices are becoming increasingly more connected. Pacemakers, defibrillators, and other medical units that keep patients alive are able to connect via Bluetooth, where they are controlled via remote controls. It is estimated that the average hospital bed has around 10-15 connected devices. Data from medical devices also are sent to outdated, centralized servers, most of which are easily exploitable. It is imperative that these devices and servers are secure in communication.\n\nIn fact, in the last year, there have been a series of global attacks made on hospitals around the world. This virus, a type called ransomware (similar to the WannaCry virus), demanded a ransom in Bitcoin in order to decrypt the hospital’s files. These files included information such as patient records, the hospital’s messaging systems, monitoring systems, and more.\n\nThis can prove detrimental to the patients in the hospital, as the doctors and nurses within various departments will suffer from communication issues. Patient identification will also be more difficult when the infrastructure is down, and is not something you want to mix up when two patients with different surgical procedures.\n\nMost hospital patient records are kept centralized and under the traditional tech, that without fail, have been breached in multiple ways over the last few years. This an lead to identify theft, as sensitive patient data can also be lost/stolen.\n\nBetween the healthcare provider, health insurance, and any investigators, data needs to be stored and verified reliably, quickly and securely. It also needs to be quick to access for all associated parties.\n<h2>Why blockchain?</h2>\n\n<hr />\n\n<b>Blockchain - immutable, secure, and distributed</b>\n\nAs stated above, blockchain is simply a convenient way to solve the above issues. It provides the necessary functions that are needed to secure and audit healthcare data, send payments, communicate with other health providers' blockchains, and a whole lot more all in one setup. It is also <strong>immutable</strong>, a key factor when maintaining the data's chain of custody.  This immutability can serve well when investigating insurance claims or malpractice investigation.  The<strong> end-to-end tracking</strong> of data makes it very easy to pinpoint issues or get a birds-eye view of each patient’s scenario.\n\nFor this article, we will be using <a href=\"nem.io\">NEM as the blockchain of choice</a> (specifically <a href=\"nem.io/catapult\">Catapult</a>). Its smart asset system, multi-signature contracts (a transaction requiring cosignatories), Apostille system, and cross-chain transfers (transfer data between two separate blockchains) make it an appealing technological solution for healthcare. In the solution portion of this article, I will go more in depth on how these NEM features can play a big role in the solution to the healthcare infrastructure.\n<h2>The solution - using NEM</h2>\n\n<hr />\n\n<img class=\"alignright\" src=\"https://cryptoconfirm.io/wp-content/uploads/2018/08/medical_graphic.png\" width=\"518\" height=\"451\" />\nWith NEM, hospitals and health insurance companies can communicate easily to each other in a secure way.  Insurance companies can keep track and have access to an immutable record of healthcare procedures.  This can save the insurance companies a lot of time and money in claims.    This tracking can be achieved by a NEM multi-signature contract could be used to hold all parties responsible and verify that that the <strong>information in the claim is true and accurate</strong>. Only those needed to be involved with the case can be given access rights via NEM multisignature.\n\nNEM’s smart asset system (often called mosaics) makes it incredibly easy to create assets that can represent the stock/quantity of an item in an immutable way, such as the number of highly regulated pain medication available. Each bottle could be outfitted with an NFC/RFID tag that is scanned once it is in use. This same concept can apply to any expendable resource in the hospital.\n\nThe NEM Apostille system could also be used to store the hash of patient files such as the letter of medical necessity (LMN). If anything went wrong during a procedure, the hash of that file can be compared to what the patient was given to ensure to was correct. This can also be check pre-operation to prevent any mix-ups. The patient's biometric can also be hashed and later verified on the operating table to avoid any receiving the wrong surgery.\n\nThe use of private blockchains networks may also work well here in conjunction with Catapult secure lock transactions (cross-chain transfers) to transfer data between two different blockchains. An example of this could be Hospital A needs to get a patient’s record from Hospital B. A transfer, with the patients permission via entering their private key or biometric, could occur from Hospital B’s network to Hospital A’s.\n\nIf a patient passes away in a hospital due to a potential malpractice, investigators will have the blockchain's immutable record of what happened to the patient.\n> <strong>Did the patient receive the right medicine? </strong>\n\n> <strong> Were the proper procedures carried out?  </strong>\n\n> <strong>Was a piece of equipment malfunctioning and crossing the threshold?  </strong>\n\n> <strong>Did the hospital neglect the patient? </strong>\n\nAll of these questions can now be easily answered because of the end-to-end tracking process that blockchain provides.\n<h2>Use of IoT</h2>\n\n<hr />\n\nRather than have ten health monitoring devices logging data to a central server, they can instead log this information <strong>directly to a Catapult side-chain</strong>. The disruptive technology of the direct transfer of data from embedded devices to blockchain, is proprietary to CryptoConfirm.\n\nThe use of IoT devices and smartwatches can also communicate directly to the blockchain under the patient's profile.  This data, such as movement, food consumption, and other fitness data will enable the health insurance company to read and provide discounts for a proven healthy lifestyle.\n\nThe data from the hospital bedside is encryptable.  In the event that one or more IoT devices can't connect, <strong>the encrypted data is stored offline to be uploaded when the connection is available.</strong>  This is to ensure that absolutely no data has been lost during any scenario where there is no connection to the blockchain.\n\nEach suite of connected devices surrounding the hospital beds can also cosign the NEM transactions to ensure a full packet of data has been properly recorded. If successful, this will signal that the devices are all operating normally under the correct thresholds, and if not, <strong>will alert hospital staff to potential issues with the equipment.</strong>\n<h2>Conclusion</h2>\n\n<hr />\n\nCoupling an immutable, secure, distributed data structure with small embedded devices creates an interesting dynamic of live, unadulterated data being stored on the blockchain.  Many lives depend on the hospital having a good data infrastructure. Embedded, IoT devices can serve as a powerful tool in making the best medical decisions for a patient.  This information can be logged on the blockchain to provide a deep, secure medical history of the patient’s stay in the hospital.\n\nThis article's goal was to introduce the disruptive ways NEM can be utilized in the healthcare industry.\n\nI hope you enjoyed this article.       "body": "<h1>Hello all, Bader here (ツ)</h1>\n<hr>\n\nAs you know, the last article I wrote was about how we could use IoT and NEM to track meat from the moment it was slaughtered to the supermarket.  I will be using much of the same principes in this example, so <a href=\"https://forum.nem.io/t/how-you-can-setup-a-basic-iot-tracking-solution-for-a-local-meat-farm-using-arduino-and-nem-iot-blockchain/17809\">be sure to check it out if you haven't already.</a>\n\nSo, this probiotic supplement just came to my door from Amazon. No, not a drink like Yalkult or Kombucha, a\npharmaceutical grade product.  \n\nProbiotics are living, breathing organisms.  They need certain conditions to survive and not activate too early within the bottle.\n\nThis led me to ask:  _How do I know that this living product is alive?  How do I know I'm getting my money's worth?_\n\nAs this came directly from the manufacturer, this is the first time I've seen one of these arrive packed with an actual cooling agent.  Usually, they arrive in a toasty warm bubble mailer straight from the Amazon FBA, basically the worst possible conditions for this type of pharmaceutical.\n\n<h1>Why should you care?</h1>\n<hr>\n\nThe entire supplement market was worth over <a href=\"https://www.statista.com/statistics/521746/market-size-vitamins-minerals-and-supplements-worldwide-category/\">$100 billion in 2017.</a>   Probiotic supplements are a niche in this market, but they are growing in popularity.  They are projected to hit a market of $7 billion by 2025.  <a href=\"https://www.grandviewresearch.com/industry-analysis/probiotics-dietary-supplements-market\">Overall, the entire market is growing and has upwards trend.</a> .  There are many people with gastrointestinal issues that need an excess of probiotics to help rebuild their gut.   In fact, there is much research that suggests that many of the diseases stems from your gut/GI tract, and is very often overlooked by medical professionals.  I know this from personal experience.\n\n\n\n<h1>The problem</h1>\n\n<hr>\nA little background about these supplements:  They contain probiotics, live bacteria that is amazing for your gut.  As with anything alive, they can only handle so much heat before they activate and die.   In the case with probiotics, it only takes some heat or humidity for them to activate.  As the environment in the bottle is not suitable for the bacteria, they die out fairly quickly.  Considering each small bottle can cost $30-$40, you do not want that happening.\n\n<h3>This leads me to believe the problem is not with the manufacturer, but rather with the transition from distributor to consumer and the conditions in which it was stored.</h3>\n\n\n<h1>A real-world example</h1>\n<hr>\n\n Recently, however, the company Natren themselves have started a Amazon storefront. This cost around $26 USD, and this is at a discount because they are just opening their Amazon store. \n\nOriginally this cost around $42 with shipping from their actual website.   They pack it with \"Dry Ice\" (spoiler: it's just a gel refrigerant, still great though).   Interestingly enough, it says on their invoice that they _have_ to be refrigerated at all times. As said before, this is to keep the bacteria alive.\n\n![DSC_0007.JPG|350x220](https://cdn.steemitimages.com/DQmcWbsbZysmHuASEbvd6MWYft3oCvPdd8njPdwU55rMpiU/DSC_0007.JPG)\n\n> Keep probiotics refrigerated at all times\n> _Pharmaceutical Grade Quality Probiotics_ \n\n<h1>Inside the box</h1>\n<hr>\nIn order to truly see if it was shipped under the right conditions (at least as much as I could see), I measured the temperature within the box and the actual supplement itself:\n![DSC_0015.JPG](https://cdn.steemitimages.com/DQmZ9PajmcRXQUoiW6VDRx4UeA1RH4YfpdWx8xGruNunNa3/DSC_0015.JPG)\n\nI took multiple readings, and the average temperature was 69°F, not bad, but not too great either.   This means that there was perhaps some loss of organisms within the bottle, but the loss wasn't too great.  **The real question, however, is how long have these been sitting in a warehouse or truck until I have received them?**\n\nFor all I know, the box of bottles could've been sitting in a hot warehouse and not stored properly until later (though if the manufacturer took the trouble to cool the shipment, it is more likely a company like Amazon did wouldn't have stored them properly).\n\n<h1>A solution - using NEM & IoT</h1>\n<hr>\n\nUsing the same technology from the <a href=\"https://forum.nem.io/t/how-you-can-setup-a-basic-iot-tracking-solution-for-a-local-meat-farm-using-arduino-and-nem-iot-blockchain/17809\">previous article, </a> we can use a SparkFun Thing and DHT11 temperature/humidity sensor combo to monitor the temperature of the bottle.  \n\nThis module communicates directly with the NEM blockchain.  There are no servers in between.\n\nOf course, it doesn't make sense to put a tracker in _every_ box shipped to your door, but it is possible to have one passively taking readings whilst in storage.   That way when you receive it in the mail, you can scan it and check if that batch was stored in the proper conditions, thus knowing if you got your money's worth!\n\n<h1>Conclusion</h1>\n<hr>\n\nThe point of these articles is to outline problems in various industries.  There are many applications for this type of variable tracker, not all of which involved temperature. Using this technology, we can provide a surefire way to measure any parameter of our choosing and upload it securely to a blockchain, where it is immutably stored.  \n\nI am going to call this whole operation this the CryptoConfirm Project.  \n\nAnd I also have a website now!
