| body |  I have been weight lifting for about 2 weeks now. I think its totally expected to say my favorite workout is benching... 😁 I tried increasing the weight I've been lifting by about 15kg 🙈. Then he said, "there's something I've been meaning to talk to a doctor about". Well, ofcourse, share your burden young man, then he said he could control his goosebumps. <hr> I was taken aback, because, I mean, that's not possible, that isn't something we can control, it's an autonomic function. So ofcourse, I'd love to see that, and asked him to show us, and he brought out his arm and gradually, goosebumps appeared. <hr> My mind was blown! Told him to take them back, and he did, then told him to bring them back, and he did! Told him to do it a couple more times, lol. It was maddening! I wanted to understand how that was possible, so I did some reading... well, let's find out what I found out. <hr> Well, for starters, the condition is called Voluntary Piloerection or Voluntarily Controlled Piloerection(VCP). Piloerection is basically the big word meaning goosebumps. Piloerection is the physiological phenomenon or response to thrill or chill, meaning the natural body's response to cold or emotions. <hr> The mechanism is a pull at the hair roots by miniscule muscles stimulated by adrenaline. Now, it should be noted that these muscles are smooth muscles and should have no voluntary control. Voluntary Piloerectors defy this rule and have good control over this phenomenon. <hr> A research was done through Facebook and I'll be making a lot of reference to it. According to the study, the participants described it as a simple physical act, like raising a leg or nodding their heads. Others explained it as flexing a muscle in the brain... please note there are no muscles in the brain, some said it was controlled by tightening a muscle behind the ear while some said the mere thought of it was enough to yield results. <hr> Interestingly, some were able to describe a pattern in which they perceived the movement of the goosebumps. The majority of participants described it to begin at the back of the neck then move gradually till it appeared on their arms. <hr> Voluntary Piloerectors, despite having voluntary control over this autonomic function, do not lose the involuntary mechanism. | body |  Our biggest problems as human ironically are so small we can't see them with our naked eyes, microorganisms! In both obstetric and gynecologic cases, we are always concerned about septic complications. Some cases get so serious and result in life long complications like subfertility. The genital tract has it's own microorganisms that are always there and are beneficial, but when a different type of bacteria gets introduced into the tract, then, we could have some problems. Let me paint you a quick picture, imagine a tube greased with jello at the top, now put your finger into the tube, now you've spread the jello into the tube! That's what happens most times in one of our headaches as gynecologists, pelvic inflammatory disease(PID). What is PID? It is a syndrome associated with the ascending spread of microorganisms from the vagina and or cervix to the sterile reproductive structures, the uterus, the fallopian tubes and surrounding structures. It is a polymicrobial infection commonly sexually transmitted. Who's at Risk? Younger ladies are more involved as it is seen majorly in ladies under 25 years. Basically, in a ladies in their sexually active phase. Ladies in unstable relationships are on this table as promiscuity increases the likelihood. Unsafe sexual practices, such as unprotected sexual intercourse exposes one to PID. Other less common modes of spread are hematogenous, as in a case of pelvic tuberculosis and spread from contiguous structures as in a case of acute appendicitis. What are the symptoms? I'm not going to bore y'all with the pathology, so let's go to the clinical features. Major features involved seen are; • Abdominal pain • Cervical motion tenderness • Adnexal tenderness Others include; • Abnormal vaginal bleeding • Abnormal vaginal discharge • Post coital bleeding (after sex) • Deep dyspareunia (painful intercourse) • Fever >38°C • Nausea and vomiting • Urinary symptoms (frequency, urgency and incontinence) It should be noted that for every 1 patient with symptoms, 2 are asymptomatic. How is PID diagnosed? PID is diagnosed clinically, with the three cardinal signs; • Abdominal tenderness • Adnexal tenderness and • Cervical motion tenderness Though diagnosis is clinical, some investigations are essential and aid in treatment. Investigations include; • Endocervical swabs • High vaginal swabs • Screen for STDs • Pregnancy test(to eliminate ectopic pregnancy) • Ultrasound scan A surgical approach may be indicated and a laparoscopy is done. Laparoscopy is the gold standard for the diagnosis of PID. How is PID treated? The treatment may be medical or surgical, but its usually medical. An antibiotics combination of ceftriaxone and doxycycline for 14 days usually does the trick. Metronidazole is added in some cases. Surgery is indicated only when peritoneal complications arise. A laparotomy is done. PID secondary to STI should be well managed by extending therapy to male partners to avoid reoccurrence. How can PID be prevented? Better sexual behavior and hygiene. Use of barrier contraceptives (condom). Screening before intrauterine device insertion. Reduced incidence of unsafe abortions. Conclusion PID is a discomforting disease and has quite a deleterious effect on the patient and long term complications such as subfertility. With better sexual practices and hygiene, if can be easily prevented. |

In both obstetric and gynecologic cases, we are always concerned about septic complications. | body |  I said I was gonna talk about this on my last post, but first, you've got to keep an open mind, and be very logical to realise the depth of what's happening around us. "Whatever makes you sleep at night" is the modus operandi of a typical Nigerian, and for good reason because otherwise one would realize how much of a nightmare we live in. We choose to sieve out or ignore the recurrent happenings in our country because of how powerless we, the people, really are. We say mean words at really terrible deeds, and that's all to it, no more is done. Let me give an instance, there's a killing of 30 random people in a place, and all that's said about it is "our government is useless!" or "God have mercy!" meanwhile these are really not as random as we think and I'll express my perspective. In the previous presidential administration, the current ruling party was so involved in expressing how incompetent the at the time ruling party was after a kidnap of about 80 secondary school children. Well, it was a big deal as it was the first time such was done in the country. Now, when we get news about massive school children abductions we just nod, it has become so rampant and exaggerated. Sometime this year, over 300 school children were abducted! Over 300! This makes you question things, this can not be incompetence, because its impossible for 50 people to move without notice, at least not in this situation, and now, 300 people missing! By the time I'm done, I'm sure you'd also realise how much of a scheme all these are and how bad and scary the situation really is. In the past 6 months, I think I've heard of more than 3 major abductions all happening in northern Nigeria, and for some reason, the security situation has not improved in all this time! Quick detour, the Nigerian citizen decided to stand up for her rights and the Nigerian government responded by sending the military in a short time to shoot her down, but there are repeated killings and kidnappings in northern Nigeria but the government is still struggling to resolve that? Okay, let's start at the beginning, after the kidnap of the 80 schoolgirls in the previous administration, foriegn military was involved in the rescue mission. After strategies were put in place with the Nigerian military and even better weapons to facilitate the mission, on getting to the enemy camp days later, the foreign personnel were able to attest that the weapons they provided the Nigerian military were found in enemy possession. That was years ago, now, with the current administration, military actions are not even considered, they negotiate with kidnappers and bandits. It sounds like a joke, but its what's happening in Nigeria! In the last 6 months, there's been about 3 abductions that the government resolved by paying ransoms worth over millions of dollars, to bandits! One of the more recent negotiations occured in enemy territory, let me expantiate, the Nigerian government had its representatives go into the enemy territory to negotiate. If you don't know the first obvious question to ask, let me help you... how did the government have the enemy's location and did not take a more dignified approach and maybe extract the children and eliminate the enemy? Instead, they paid millions of dollars further empowering the enemy. Oh, okay, you think they didn't take military action because of their concern for the children? Then why have they not increased the state of security of the region in 10 years?! If the lives of your citizens matter so much to you, then why not do the bearest minimum and put measures to protect your citizens! Their fundamental right! In the last 6 months, the Nigerian government has spent several millions of dollars in ransom. That's what the Nigerian people would tell themselves, but if we get logical, this "enemy" had government issued military grade weapons, this "enemy" keeps operating in a certain region and yet, the government hasn't intervened and it's been at least a decade. This "enemy" keeps getting paid by this certain people in the name of ransom. In my opinion, these aren't ransoms, they are government funds being redirected to a private militia. In simple words, there's a big thing brewing up and it's getting massive funding, and there's nothing we're doing about it but pretending we don't see it. |

There are so many complex and understood mechanisms in the body and some result in abnormalities and irregularities. Okay, let's talk about quite a common one, Polycystic ovary syndrome(PCOS). Introduction. Polycystic ovary is a syndrome of ovarian dysfunction characterized by; • Anovulation • Hyperandrogenism • Polycystic ovary morphology Okay, let's break these down for simple comprehension. Anovulation is when a woman undergoes a complete cycle without the release of an ovum(egg). Hyperandrogenism is a state of increased levels of steroid hormones, typically found in males, such as testosterone. This feature is responsible for the Male pattern hair distribution seen in such patients. Polycystic ovary morphology is a feature seen on ultrasound or any applicable imagine technique. Aetiology The cause of this syndrome is unknown. But some factors have been implicated and includes; • Abnormalities in estrogen and testosterone metabolism • Hyperinsulinaemia as a result of increased insulin resistance, which causes the ovaries to produce more androgens. • Abnormal function of the hypothalamic-pituitary-ovarian(HPO)axis. The HPO axis is responsible for regulating the gonadal hormones, that is, estrogen, estradiol, prolactin and androgens. Epidemiology 5-10% of women aged 15-44 years, the child bearing period. Clinical features • Asymptomatic • Oligomenorrhoea or amenorrhoea • Anovulation • Hirsutism • Obesity • Subfertility • Recurrent early miscarriages • Acanthosis nigrican For more details, click here Diagnosis • Elevated serum levels of the following hormones; Testosterone Leutinizing hormone & follicle stimulating hormone Free androgen index • The following may be slightly elevated or normal; Estrogen Fasting Insulin OGTT • The following have low serum levels; Sex hormone binding globulin • Transvaginal ultrasound scan Shows features of a polycystic ovary morphology which includes; • >12 subcapsular follicular cysts, • <10mm in diameter and • an increased ovarian volume of>10mm³ Diagnosis us made using the Rotterdam criteria. • Chronic anovulation(anovulation of >6 months) • Hyperandrogenism evidenced by hirsutism or voice changes • Polycystic ovary morphology on ultrasound sonography Treatment There is no definitive treatment for PCOS and treatment principle is directed towards rectifying the symptoms. • Treatment of Amenorrhoea Oral contraceptive pills Oral cyclical progesterone • Treatment of subfertility Ovulation induction agents; Clomiphene citrate Tamoxiphene Metformin Gonadotropins Laparoscopic ovarian drilling • Treatment of hirsutism Medical • Cytoperone acetate • Metformin • Flutemide • Ketoconazole Physical • Depilatory cream • Hair plucking • Laser treatment • Electrolysis • Treatment of obesity Diet Exercise Complications • Diabetes mellitus • Dyslipidemia • Hypertension • Cardiovascular diseases • Endometrial cancer • Breast cancer No evidence to suggest that prophylactic treatment is beneficial. Follow up is recommended. Conclusion PCOS is a common disorder in women of reproductive age and they mostly present with complaints of subfertility. | body |  When do we agree certain things are not random coincidences or results of incompetence but actual well thought out actions to serve a long term agenda? Let me start from the obvious and then get into the very conspicuous we pretend to allude us. Let's talk about the sources of revenue in Nigeria, okay, the relevant one, Crude oil. Since I was a completely ignorant child, I learnt Nigeria is rich in crude oil and it's our major source of revenue. But for some reason, we didn't have what it took to refine this crude oil and so we sold cheap crude oil and bought expensive petroleum products. Didn't make any sense to the ignorant child I was, and now, I'm a less ignorant person and it's been what, 20 years? Same shit, different day! Nigeria still does the illogical thing and sells cheap to buy expensive products when an option of building a functional refinery is constantly ignored like its not the real solution. Then, finally, a refinery is being built, but wait for it, it's a private property! The crude oil products that the nation should use to better her economy is now basically given to a private cooperation, ergo, the revenue made will majorly go to a particular sector of the population. So, the wealth of the nation continues to go to the affluent few and the people live in poverty. Let me take a quick detour, drove past a pregnant woman having a little child on her back one time in Bayelsa, she was drinking water from a muddy pond. That was a major trigger! For clarity, Bayelsa is one of the states in the country that's rich in crude oil. You can not fathom the level of poverty her people live in. And when the ruling class finally try to take a step in the right direction, they do it only to make themselves richer. The people asked her government to solve the problem of police brutality, her government sent in the army to silent them. When the people strive to do better, they get arrased by their government, when they speak up, the get beat down by their government. These aren't random actions by a ruling class who dosen't know what its doing. It's the exact opposite! These are carefully planned out actions by a group of people who know exactly what they're doing. So now, I pose the question to you, where do we go from here? In my country, a citizen was able to use the resources around him to build a company worthy of recognition and respect, a vehicle producing company indigenous to Africa! The company is currently functional, but you won't believe that there were law suits to keep that from happening. There is a constant need to stop progress in this place by the affluent, and I don't understand why, because another's prosperity doesn't hinder yours! "body": "\nWhen do we agree certain things are not random coincidences or results of incompetence but actual well thought out actions to serve a long term agenda?\nLet me start from the obvious and then get into the very conspicuous we pretend to allude us. Let's talk about the sources of revenue in Nigeria, okay, the relevant one, Crude oil. Since I was a completely ignorant child, I learnt Nigeria is rich in crude oil and it's our major source of revenue. But for some reason, we didn't have what it took to refine this crude oil and so we sold cheap crude oil and bought expensive petroleum products. Didn't make any sense to the ignorant child I was, and now, I'm a less ignorant person and it's been what, 20 years? Same shit, different day! Nigeria still does the illogical thing and sells cheap to buy expensive products when an option of building a functional refinery is constantly ignored like its not the real solution. Then, finally, a refinery is being built, but wait for it, it's a private property! The crude oil products that the nation should use to better her economy is now basically given to a private cooperation, ergo, the revenue made will majorly go to a particular sector of the population. So, the wealth of the nation continues to go to the affluent few and the people live in poverty. Let me take a quick detour, drove past a pregnant woman having a little child on her back one time in Bayelsa, she was drinking water from a muddy pond. That was a major trigger! For clarity, Bayelsa is one of the states in the country that's rich in crude oil. You can not fathom the level of poverty her people live in. And when the ruling class finally try to take a step in the right direction, they do it only to make themselves richer.\nThe people asked her government to solve the problem of police brutality, her government sent in the army to silent them. When the people strive to do better, they get arrased by their government, when they speak up, the get beat down by their government. These aren't random actions by a ruling class who dosen't know what its doing. It's the exact opposite! These are carefully planned out actions by a group of people who know exactly what they're doing. So now, I pose the question to you, where do we go from here?\nIn my country, a citizen was able to use the resources around him to build a company worthy of recognition and respect, a vehicle producing company indigenous to Africa! The company is currently functional, but you won't believe that there were law suits to keep that from happening. There is a constant need to stop progress in this place by the affluent, and I don't understand why, because another's prosperity doesn't hinder yours!\nAbout the very conspicuous we pretend to allude us, I'll discuss in my next post.",

The woman is faced with so many challenges, her social challenges transcends to be her medical predicament. What do I mean? The general standard of living for women in Africa is undoubtedly poorer than anywhere else in the world. To start with, as a growing child, the girl child isn't adequately educated because of her perception by the community, which results in poor health seeking behavior. The woman with a medical condition would rather stay back hoping the disease subsides or sees an unqualified personnel who usually doesn't have the necessary skills to help her. But let's talk about what is bothering me, Uterine prolapse. Other than the medical implications of uterine prolapse, the social consequences are quite dire. The patient may end up being an outcast in her community and a lot of cases end up in broken marriages and families. The woman may then tends towards options she'd rather not take like prostitution since she's been pushed to the wall. Introduction. Genital prolapse is the downward displacement of the uterus or cervix through the vagina. Its as a result of weakness of the uterine support. Epidemiology. It is more commonly seen among women aged above 54 years of age, it is one of the most common reasons for undergoing a hysterectomy. Aetiology. It could be congenital, where there is weakness of uterine support from birth. Or it could be acquired. This is the more common case. Conditions that could result in weakening of the uterine support include; • Repeated childbirth • Difficult labour • Instrumental deliveries • Macrosomia(big baby) • Perineal tears • History of pelvic surgery • Post menopausal atrophy • High body mass index • Chronic increased abdominal pressure; chronic cough, long history of farming and heavy load carrying. This is why it's important to educate and properly cater for the girl child. Classification. For simplicity, genital prolapse can be classified into 3 types; • First degree; the cervix is still within the vagina • Second degree; the cervix is at the vagina introitus • Third degree; the cervix extends beyond the vaginal introitus Symptoms. Some of the symptoms seen in genital prolapse include; • Feeling of pressure in vagina • Feeling of something coming down the vagina • Urinary incontinence • Urinary frequency • Urinary urgency • Lower back pain • Difficult sexual intercourse • Constipation Diagnosis. Diagnosis is done clinically by confirming the symptoms and by performing pelvic examinations. An ultrasound scan or an MRI may be helpful in accessing the level of severity. Treatment. Treatment options include; • Non-surgical approach 1. Pelvic floor exercises for younger women- kegel's exercises. 2. Vaginal pessaries for older women. • Surgical approach 1. Manchester repair. 2. Colpocleisis The above are not definitive treatments and reoccurrence is a likelihood. The definitive treatment for genital prolapse is Hysterectomy with pelvic floor repair. Prevention. • By performing Kegel exercises regularly and correctly, the pelvic floor is strengthened and the incidence of prolapse is reduced. • Preventing and treating risk factors such as constipation. • Avoid heavy lifting or using correct body mechanics whenever lifting is deemed necessary. • Proper management of chronic cough. • Its important to maintain a healthy weight through a healthy diet and proper exercises. • Estrogen replacement therapy may be considered during menopause. • Horse Chestnut, Saw Palmetto, Oats, Equisetum, Elecampane and Couch Grass as well as the Bach Flowers Vine, Oak , Raspberry Leaf, Sweet Chestnut and Hornbeam. Together these herbs work to strengthen muscles and ligaments and support the structures and organs of the pelvis. Conclusion. Uterine prolapse is a condition that has serious medicosocial implications as so many of the patients end up losing their homes and marriages. | body |  Why is Africa not doing as well as other continents? Why do we need aid from foreign countries? Why is the leading cause of death in children hunger and malnutrition? Why are easily preventable diseases so rampant in our communities? Does the black man lack the necessary skills to grow all by himself? Is he not just as adequate as his foreign counterparts? I'll say just one thing. He is. But he is too selfish and shortsighted. From my experience, he's more interested in what a complete stranger thinks than what his brother thinks. Why do I say this? Lemme bring you home. In Nigeria, the affluent drivers on the same roads as every other person, but believe me when I say, even animals would have a though tome walking on these roads. This same affluent takes a trip to a foreign country and spends more money than even the foreigners have! His home is in shambles but he's on vacation in a foreign land doing irrelevant things for irrelevant reasons to irrelevant people. Makes no sense! I keep wondering why these things keep happening here. I now have an idea... we let them happen! We are so concerned about keeping the little we have that we forsake the bigger picture for momentary relief. Plot twist, the little we struggle to keep will still be snuffed out and we'll be left with absolutely nothing! And then, it would be too late to even do anything about the problem. Its crazy, I feel helpless, they keep increasing the prices of things, keep reducing income and keep devaluing the currency. I used to think it was incompetence, but now, I know it's the exact opposite. Its a scheme, a plan to keep he people down. It's the year 2021 and we do not have basic amenities in Nigeria. Electricity is not constant, and growing up, I used to think it was a work in progress. I'm an adult now, and there's still no progress. Countries have had stable constant electricity for decades now, so even if you don't know how to create it, how difficult is duplicating it? How difficult is it to make sure the roads are adequate and that every citizen has food to eat?! I'll tell you what this all is... 21st century slavery! I had a conversation with a friend of mine, and at the talk, we made a conclusion, the black man is the black man's problem, but thinking foreign bodies have the black man's best interest is a big joke. Recently saw the movie Judas and the black messiah, all I see is history playing over and over. Let's rewind even further, the initiation of slave trade, the black man happily sold his kin. But this trade was done with the foreigners, and the both of them are still in business till today. The affluent black man and the foreigner. To answer my initial questions, I'll be plain and simple. Africa is wrecked and wretched today because of the greed of the few affluent, but more importantly, because of the fear of losing what little he has, the poor black man. |
